Welcome to the Waiver Request Form for the Las Vegas Fly-Up Trip! The cost for this trip is $99, but you may be eligible for a waiver if you are a high school junior or senior who meet one of the following criteria:

• You received an ACT score of 26 or above
• You received an SAT score of 1230 or above
• You are in the top 10% of your high school class (seniors only)
• You are currently enrolled in Collegiate Academy

To successfully receive your flight ticket, you must complete the following steps:

1. Complete the Nevada Bound waiver form below.
2. Submit your supporting documentation that verifies your eligibility.
3. Register for your first choice trip date (skip the payment portion) using this link: Register Here

Note: This form is only a request for a waiver. All submitted documents and your registration will need to be reviewed before we can proceed with purchasing your airline ticket. Failure to follow all steps in the process may result in you not receiving a waiver.
